Here are guidelines before the flight.

Do not transfer a critically ill or unstable patient in an aircraft.
Do not get involved in any activity that will lead to a medico-legal case or harms.
Be prepared in case a new medical emergency arises in flight.
If a patient had a fracture that has been fixed and vitals are normal for more than 48 hours after fixing the fracture, he or she is not a critically ill or unstable patient. There are various similar scenarios.

Here are guidelines while in flight.

If there is a medical emergency in flight and the patient needs emergency diagnosis, monitoring, and treatment, where should the patient be located in flight?

Most aircraft do not have space or location reserved for a medical doctor and patient in case a crew member or passenger becomes ill or incapacitated.

If a pilot dies during the flight, another pilot can take over.
The aircraft is on cruise control.
